Here is an email I recieved

Could possibly be caused by bad mix, Aerating additive added to lime based mortar mixes ,etc
A. Air bubbles in mortar weakening strength and durability
B. Air voids enlarge
C. Chunks of mortar errode and fall out
D. Eventually wall becomes unstable
Repointing should be carried out at C well before D
